Tele-ophthalmology is one of the branches of the telemedicine that provide eye care services through using digital medical equipment and telecommunications technologies. Diabetic retinopathy is one of the causes of the preventable blindness in the world and also is one of the diseases that can be diagnosed at the beginning stages and treatment recommendations can be presented by using tele-ophthalmology

To identify the necessary data elements and system capabilities, the necessary information was obtained by using the researcher-made questionnaire and then the tele-ophthalmology system was designed using Visual Studio and ASP.NET and was analyzed through using the Standard Usability Testing Questionnaire to evaluate the users' satisfaction.

This system included patient's demographic and clinical information, retinal images and capabilities for users to diagnosis of diabetic retinopathy. The mean scores of usability testing results were 7.3, 7.1 and 7.3 out of 9 for ophthalmologists, residents and optometrists respectively, suggesting that the usability was at a good level

This system can be used to diagnose diabetic retinopathy in order to improve the accessibility of the healthcare services for those who are in the remote areas or cannot afford the related medical expenses or the areas which do not have enough access to ophthalmologists.
