Purpose:
|
To study 5-year changes in visual acuity (VA) and incidence of visual impairment (VI) in the middle-aged Iranian sample of the Shahroud Eye Cohort Study.

Methods:
|
Of the 5079 Phase I survivors, 4737 (93.3%) completed their 5-year follow-up. Age- and gender-specific incidence rates of VI were deduced based on uncorrected VA (UCVA), presenting VA (PVA), and best corrected VA (BCVA). Risk ratios (RR) of incident VI were calculated using log-binomial regression.

Results:
|
Prevalence of VI by PVA (2.29% vs. 2.07%) and BCVA (0.71% vs. 0.53%) was lower in Phase II. The incidence of VI was 1.12% (95% CI: 0.82-1.42) by PVA, and 0.19% (95% CI: 0.07-0.32) by BCVA; leading causes were uncorrected refractive errors and diabetic retinopathy. Risk factors for incident PVA impairment were older age (RR=1.05, p=0.044), lower education (RR=0.89, p=0.002), and being diabetic (RR=3.74, p<0.001).

Conclusion:
|
This study is the first to document the incidence of VI in an Iranian population. Incidences were low mainly due to the relatively young sample population as well as the influence of the study on participants’ eye care seeking behaviour. As less treatable causes begin to surface, the demand for low vision clinics should be noted for proper health planning.
