Purpose:
To report a case of endogenous endophthalmitis after severe burn injury
Methods:
A 12-years-old girl admitted due to a full thickness burn in 46 percent of TBSA. She underwent serial excision and graft. She treated with broad spectrum antibiotics because of severe sepsis. In twenty third days of admission, she complained of pain and decreased visual acuity in her right eye.
Results:
Ocular examination revealed pan- uveitis associated with dense vitritis and large chorio-retinal abscess located at the macular area in the right eye. The left eye was completely normal. Smear and culture of vitreous biopsy revealed candida albicans. With diagnosis of fungal endogenous endophthalmitis , the patient was treated with systemic fluconazole , intra-vitreal injection of Amphotericin B and pars plana deep vitrectomy and silicon oil injection. After 1 month ,final ocular examination revealed no significant change in BCVA due to extensive macular scar ,although intraocular infection disappeared completely.
Conclusion:
Early diagnosis of endogenous endophthalmitis in high risk patients with severe burn injuries , could prevent severe visual loss .
