Purpose:
To assess the efficacy and safety of systemic propranolol for periorbital and orbital capillary hemangioma
Methods:
Participants: Newly diagnosed cases of periocular capillary hemangioma in children aged up to 24 months.
All patients were treated by oral propranolol at a starting dose of 0.5 mg/kg/day, which was gradually increased up to two mg/kg/day. The treatment was continued for six months for local lesions and for eight months for diffuse lesions. Follow-up was continued for one year. Parents were asked to report possible side effects at each visit.
Results:
Thirty-two patients (7 males, 25 females) with a mean age of 4.5 months (range: 1.5 to 18 month) were included. Five patients had periorbital lesions extending into orbital space as documented by orbital CT or MRI. Sixteen patients (50%) showed excellent response to the therapy (defined as a decrease greater than 75% in lesion size or color intensity), 15 patients (47%) demonstrated partial response (defined as 50% to 75% decrease in size or color intensity) and one patient (3%) showed no response (less than 50% reduction in size or color intensity). No significant side effect was reported.
Conclusion:
Oral propranolol is a safe and effective option in the treatment of periocular capillary hemangioma.
