Purpose:
|
As some patient needs to have scleral fixation IOL we can do corneal fixation with more benefit and lower risk.
|
Methods:
|
First it was done in patient with about 180 degree capsular support at one side and one with no capsule. one needle of Tow double armed 10/0 sharp needle goes through the snap corneal incision which is like phaco incision then it goes though iris at periphery and comes out through pupi and back out of incision .The needle is cut and it is tied to IOL hoptic. The IOL is inserted in the globe behind iris .The other needle is tied to the internal lip of incision so that IOL is in good position. The same is done for other hoptic and fixed 180 away from the first .wounds are sutured.

Results:
|
IOL stability in 2-5 years follow up is good by corneal refraction and UBM. Pupil shape and activation is good. No more obvious endothelial cell loss .No iridocorneal adhesion.

Conclusion:
|
It is comparable with sclearal fixation IOL .it is easy, time consuming, with no risk of conjectival and sclearal destruction or blind sclearal perforation.
