Purpose:
To determine the efficacy of BTX-A on the course of acute sixth nerve palsy compare with natural course of the palsy.
Methods:
Patients with duration of palsy less than 3 months were enrolled randomly in the study in two groups of injection and observation and then a matched patient in terms of age and cause was selected for another group. Diplopia, deviation and limitation of adduction were compared after 6 months. In this study 28 patients with sixth nerve palsy, less than 3 months, were randomly divided to injection and observation groups. Deviation, limitation of abduction and diplopia were compared after 6 months.
Results:
Twenty eight patients divided in two case and control group.Diplopia decreased in the case group significantly more than the control group (p=0.04). Esotropia was decreased in case and control group -21.82pd and -12.67 pd respectively(p=0.003) Severity of the limitation of abduction were the same in two group.Etiology, duration, age and sex did not affect the improvement. Abduction limitation of three minus and more were common in poor responder patients in both groups(p=0.05)
Conclusion:
: Although the BTX-A injection resulted in improvement of deviation and diplopia, it was not as effective against the abduction limitation
