Purpose:
Diffuse lamellar keratitis is a sterile inflammatory reaction known as a common complication post LASIK.
This case report represents clinical course of a DLK after repositioning of traumatic displaced LASIK flap.
Methods:
A 59 year-old woman was struck in the right eye by the edge of a wooden material 15 years after having LASIK. The injury resulted in partial displacement of the LASIK flap. The LASIK flap was rolled on itself from 7 to 12 o’clock. The patient developed DLK 2 days after the flap was repositioned. Topical and oral corticosteroid and antibiotic priscribed and by day 6, the clinical evidence of the DLK had resolved, and by day 9, uncorrected visual acuity was 20/20.
Results:
Eye trauma 15 years after LASIK can result in displacement of the LASIK flap, and DLK can develop following flap replacement. Long-term anatomic and visual results are usually good.
Conclusion:
In conclusion this report highlights two important concerns; first we should expect occurrence of DLK in any condition that post-LASIK cornea is involved by trauma or infection. Second any kind of trauma could result in flap displacement or tear even so many years after LASIK.
