Purpose:
|
To evaluate the results of phacotrabeculectomy with releasable suture in patients with glaucoma and cataract in Shafa Medical Center of Kerman
|
Methods:
|
In a longitudinal study 36 eyes were evaluated . Phacotrabeculectomy wasdone because of uncontrolled IOP or nonreliable patient for medical therapy and significant cataract, or low VAdue to cataract and simultaneous glaucoma.
Trabeculectomy was done with releasable sutures+Mithomycin-C.patients were examined in postoperative days 1, 2, 3, 5, 7, 14, 21 and 30.then examination was every 2-4 weeks until 3 months and then every 3 months.
Releasable suture was removed in first 3 weeks if IOP more than 12mmHg.

Results:
|
The age was 22-82(66.6±11.9)years. Follow up was 16.63±12.27 months.
Preoperative IOP was 12-52 (28.38 ±9.36)mmHg. Postoperative IOP was 5-22(12.05±3.96)mmHg (P<0.0001).
Preoperatively patients used 2-4(2.58±0.09) drugs, postoperatively they necessitate 0-2 (0.58±0.15) drugs.(P<0.0001).
11 eyes (30.55%) had early postoperative complications: flatAC , POU, leakage, choroidal effusion and vitreous incarceration.
After surgery IOP lower than 21mmHg was seen in 35 eyes (97.22%), under 16 mmHg in 31 eyes (86.11%) and under 12mmHg in 21 eyes (58.33%).
Procedures following phacotrabeculectomy were subconjuctival 5FU in 3, AC formation in 2, resuturing 2, choroidal tap in 1.

Conclusion:
|
phactrabeculectomy with releasable suture is effective and safe procedure for coexistant glaucoma and cataract.
