Purpose:
|
The purpose of this presentation is to report a case of inadvertent vitreous staining with trypan blue during phacoemulsification in a patient with anterior megalophthalmus.
|
Methods:
|
preoperative findings in the patient with mature cataract including high IOP, deep anterior chamber, small pupil and high myopia unveiled the diagnosis of anterior megalophthalmus. Phacoemulsification was performed via a routine technique of white cataract with phakodonesis including using iris retractors, CTR and administration of trypan blue for capsular staining. Surprisingly, in defiance of complete nucleus fragments removal, the red reflex was absent. Intraoperative indirect ophthalmoscopy was performed and retina was not visible.

Results:
|
On the first postoperative day, the corrected distance visual acuity in the right eye was 2/10 and no red reflex was detected. Visual acuity improved day to day and only after 7 days postoperatively, the normal fundal red reflex appeared and retina was completely visible and attached.

Conclusion:
|
In this case presence of zonular dehiscence due to anterior megalophthalmus, high myopia and phakodonesis, explaining entrance of vitreous in the anterior chamber and Inadvertent staining of the vitreous with trypan blue, that gained access to the vitreous cavity from zonular defect, is account for mentioned events.
