optical pathway glioma (OPG) rarely involves intraocular structures. We present a 44 years old man with optical pathway glioma (OPG)in left orbit with extension into the Globe.

The vision in the eye was no light perception;and a Proptosis of about 2-3 mm was present.Computed tomography and MRI of brain/orbit was done that Showed a fusiform enlargement of left optic nerve and total filling of vitreous cavity by a mass that has high intense signal in T2 weighted images in MRI that is a property of OPG .surprisingly, there was no extention to intra cranial cavity.

The therapeutic plan was enucleation ;pathologic evaluation of the mass and globe showed: spindle cells with eosinophilic cytoplasmic structures ,and areas of coarsely reticulated and myxomatous area that is in favor of OPG.

The remarkable feature of our case is the rare large intra ocular extension of OPG in a case that had No intra cranial extension .we hope that this presentation improves our insight about both OPGs and intraocular tumors.
