Purpose:

To evaluate the effect of repositioning continuous intrastromal corneal ring toward the apex of the cone in keratoconic patients who are not satisfied with their present vision.

Methods:

This prospective study comprised patients with history of MyoRing implantation for the treatment of keratoconus who were not satisfied with their vision and had decentered cone entered the study. 35 eyes of 35 patients were included. We repositioned the MyoRing 0.5 mm toward the apex of the cone according to pentacam imaging and evaluated the changes in refraction, topography and aberrometery one month after repositioning.

Results:

Mean uncorrected distance visual acuity were significantly increased (P=.04) from 0.74±0.54 preoperatively to 0.46±0.25after surgery. Mean spherical equivalent refractive error did not change significantly but mean astigmatism decreased from -4.3 ± 1.52 to -2.8 ± 1.9 D after one month. The RMS values for secondary astigmatism, higher order, and coma aberrations were significantly reduced with surgery (P < 0.05).

Conclusion:

This report showed that MyoRing repositioning toward the apex of the cone might be considered an acceptable promising option in keratoconic patients who have poor visual and refractive correction after MyoRing implantation.
