Purpose:
|
To evaluate refractive outcome of penetrating keratoplasty (PKP) and deep anterior lamellar keratoplasty (DALK) for keratoconus with same size punch and trephine sets.

Methods:
|
In this study, 35 patients underwent corneal transplantation by PKP and 35 patients by DALK. We used Katena Barron vacuum punch/trephine sets. Diameter of punch and trephine were chosen the same in all patients. PKP performed with 8 separate and 1 running 10-0 nylon stitches. Baring of Descemet’s membrane in DALK group carried out with big bubble technique. Patients evaluated 3 month after removal of all sutures.

Results:
|
Median corrected acuity in both PKP and DLK groups were 0.5 decimal notation. 17.1 % of PKP group and 25.7 % of DLK group had corrected acuity equal to or better than 0.8. 77.1 % in PKP group and 68.6 % in DALK group had acuity between 0.8 and 0.3.. Mean spherical equivalent refractive error in PKP and DALK groups was -3.86 +/- 3.72 and -2.78 +/- 4.19 diopters respectively. The difference was not statistically significant between two groups.

Conclusion:

Both PKP and DALK performed with same-size punch/trephine sets are acceptably effective methods for treatment of advanced forms of keratoconus.
