Purpose:

To report a new method for treating complicated wound gapes in radial keratotomy

Methods:
|
A 48-year-old woman who had undergone radial keratotomy (RK), 15 years ago was referred to our clinic because of severe redness and photophobia.Slit photograph showed sectoral corneal edema and deep focal gray-white corneal infiltration as well as peripheral satellites in that sector associated with an 1*1mm epithelial defect and ciliary injection.Corneal smear showed fungal hyphae documented as Fusarium species found by culture. Confocal scanning of that segment showed fungal hyphae in that segment in stromal sections.

Results:

We removed the edematous corneal sector and put remained tailored corneal lenticle for descemet stripping endothelial keratoplasty (DSEK). After being sure about the size and shape, the corneal tailored segment was put on place and fixating of the segmental corneal lamellar graft was performed with 10-0 nylon sutures. The patient`s best corrected visual acuity reached to 20/20 one week later.

Conclusion:
|
Using tailored corneal lamellar keratoplasty seems to be a promising method for treating complicated corneal gapes in patients who have previously undergone radial keratotomy (RK). Further studies using this method to evaluate its long term results are needed.
