Methods:

Phacoemulsification was planned and the anterior chamber was entered with 2.4mm clear corneal superotemporal incision. As the anterior chamber was entered, Liquified cortical matter was then removed by irrigation and aspiration probe then lift up the morgagnian nucleus into anterior chamber which was then phacoemulsified by horizontal phaco chop technique.

Results:

Difficult step of cataract surgery in these hypermature intumescent cataracts is performing a capsulorhexis or continuous curvilinear capsulorhexis (CCC) without an extension upon the opening of the capsular bag.

Conclusion:
|
An intact CCC is critical, and careful attention to technique must be paid in order to prevent splitting or running out of the anterior capsule. Once an adequate capsulotomy is performed, the difficulty of the remaining surgical procedure depends on the hardness of the lens nucleus.
